Fundraising Notification Form

This form is utilized to inform the Saugerties Youth Hockey Association Executive Board of all fundraising taking place throughout the league.  Please fill out a separate form for each individual fundraiser or fundraising event.  

Please note, all income and expenses for fundraising must go through the SYHA Treasurer.  SYHA is responsible for maintaining proper financial records for IRS reporting.

Please submit a Fundraiser Notification form whenever there is a fundraising event or when leaving any fundraising proceeds in the drop box.  This allows for proper allocation of funds.


Price Chopper/Market 32 is a proud, longtime sponsor of youth sports.  Their corporate philosophy connects deeply with the inherent wholesomeness, spirit of teamwork and healthy competition.  To that end, Saugerties Youth Hockey Association is pleased and excited to partner with them in our fundraising efforts!

 

The “Big League Hockey Raffle” offers the following prizes:

 

  • $500 Price Chopper Gift Card

  • $250 Stub Hub Gift Card

  • $100 Shop.NHL.com Gift Card

  • $100 Amazon Gift Card

  • $100 Nike Gift Card

Each ticket sells for $5 and 100% of the proceeds go directly to Saugerties Youth Hockey!!

The drawing will be held on Wednesday, February 16th, 2022 and winners will be contacted by either phone or email.

 

Here’s how the fundraiser works:

 

  1. Every rostered SYHA player will receive a pack of 10 tickets to sell.

  2. The ticket purchaser must fill in the ticket stub (you keep the stub, they keep the ticket).

  3. We can accept cash or checks.

  4. All ticket stubs and payments (and unsold tickets) must be returned in the provided envelope to the SYHA drop box (next to Kiwanis Pro Shop) at any time but no later than February 9, 2022.

  5. We will process all envelopes and submit to Price Chopper/Market 32 for the drawing on February 16, 2022.

  6. Winners will be contacted directly!

☆This is an organization-wide fundraiser, and as such, we encourage full participation!  As an added incentive, the team with the highest percentage of sales will receive a team pizza party!  Additional tickets are available upon request.
