Expense Reimbursement and Fundraiser Notification Forms are available by clicking on the link below. You can also download the PDF and print it, or pick up a paper copy of either form at the SYHA drop box at the rink.
Prior to receiving reimbursement, a request form must be filled out either online or in hard-copy. All reimbursements are subject to approval of the SYHA Executive Board if the expense was not pre-approved.
Please submit a Fundraiser Notification form whenever there is a fundraising event or when leaving any fundraising proceeds in the drop box. This allows for proper allocation of funds.

This form is utilized to inform the Saugerties Youth Hockey Association Executive Board of any expenses incurred on behalf of SYHA that require reimbursement.

This form is utilized to inform the Saugerties Youth Hockey Association Executive Board of all fundraising taking place throughout the league. Please fill out a separate form for each individual fundraiser or fundraising event.
Please note, all income and expenses for fundraising must go through the SYHA Treasurer. SYHA is responsible for maintaining proper financial records for IRS reporting.
